Alterations to name/s may be included in existing passports of Sri Lankan nationals provided that the name/s in the birth certificate has/have been altered in accordance with the birth registration regulations of Sri Lanka i.e. those persons whose birth has been registered in Sri Lanka can fill the relevant application form for alteration of name together with 3 recent documents indicating that the person is using a different name and requesting that the name be altered accordingly. The relevant form which is available on this website could be advisedly sent to the Registrar of Birth in the place, where the applicant was born and requesting approval to alter the name

Female applicants using name of her husband after marriage, who wish to include it in her passport should fulfill the following formalities.

Required documents ;

Original or notarized copy of marriage certificate issued in Sri Lanka or in a Foreign Country with two photocopies. The marriage certificate should be certified by the Consular Division of the Ministry of Foreign Affairs, Colombo. If the marriage has been registered outside Sri Lanka , the Marriage Certificate should be countersigned by the diplomatic representation of the respective country.
